Low frame rate regardless of Settings.

I have a GTX 1080 Ti and no matter what I put the graphics option too I have high frame times and low fps. Even if I set the resolution scaling to the lowest possible setting so it's all blocky and all effect options off.
The GPU and CPU usage remain low.
How do I resolve this?
