Heres the setup I use. Not sure it'll work for you but hopefully it helps.
X=acceleration
sq=brake/reverse
O=hand brake
Triangle=nitrous
D pad = L/R turning---up/down are steer up/down for the air weight transfer
L Trig= Weight transfer
R Trig= Flash lights
I found a better solution, that fixes the turning problem. There was a user created controller setting profile for the xbox 360 controller, but I was able to set it up for the ps4 controller. After making minor adjustments, it works just as good as I remember it back when I played it on my ps2. :) If you want to try it out yourself, plug in your controller then go to the links for the game, then go to to Controller configuration > Browse Config > Community. The profile should be right there.
Hope this helps someone else.
P.S. I wasn't sure which of the controllers I should have supported from the Steam settings so I have both PS4 and XBox configuration boxes checked.
