Percentage is the first line IIRC.
Off-Road : there are four : two with a Patriot, one with a Landstalker and one with a Stallion.
Payphones : also four different payphones in the game : El Burro (blue), King Courtney (green), D-Ice (red), and Marty Chonk (hidden, missions start at the payphone north of Joey's garage).
If you don't remember having passed one of all of these missions, then that probably solves your problem.
Remote-controlled : where you explode gangcars using a RC Toy provided by a van, found in Mafia / Diablo / Yakuza / Nines territory.
Only "fix" to this issue is to use a save editor (there are a few on Google, and are usually well made) to pass the missions that were skipped. If you don't want to use external software, the only way is to start a new game from the very beginning.
If you are going for a new game, here are the missions you should do in priority in order to prevent yourself from being locked again :
- Luigi's and Joey's missions before starting Salvatore's missions.
- Kenji's missions before starting Donald Love's missions.
- Passing Uzi Rider (D-Ice's first payphone mission) before completing the storyline (The Exchange from Catalina).
