The Donald Love mission where you have to drive the security van with max wanted level has caused me quite a few failures too.
The game isn't that hard, it can be buggy and that can screw you over sure. Play GTA 1 if you want to really pull your hair out, one hit kill and you can't retry missions. Not to mention missions are best for the score multiplier needed to move to the next level.
GTA 3 is a challenge, but it is still solidly made for the most part.
Sometimes you can mess up by doing missions in the wrong order, like killing someone in one mission but you still have missions to do for them so you can't get 100%. Then there's El Burro that has you drive around Portland picking up magazines, best done before the mafia hate you...although it is doable while they do, you have to drive perfectly and hope they miss a lot of their shots.
Same, this one always gives me a headache. Sometimes he gets shredded immediately, other times he makes it all the way to the garage only to end up dead as he pulls up to it. Keeps getting stuck too...but then escort missions are just trash anyway.
