The key difference right now is that the paid version supports multi-section arrangements. You can have multiple sections with different key and time signatures, and different tempos in the same song.
There are features in development that will be part of the paid version before it is out of Early access: custom effects (delay, reverb, chorus, etc.), custom sounds and images (a subset may end up in the free version).
Songs created in the full version are shareable online, but won't be editable without owning the full version.
There will also be smaller improvements and bug fixes in both the free and full version as development continues.
