It is in the entrance area at the very first elexir well.... but it is bugged.... you do not get the lore record (perhaps if you have already page 5)..... I have the same problem....I have found all other lore pages but this one is still missing because of the bug

It's called "On Elixer Well, The Springs of the Elixer" and I can open it, but it doesn't register in the Lore pages.

I have checked this. And no, it didn't get fixed. The bug is still there.
I went to the lore pages location with my main char, reading the page still doesn't unlock it in your journal.
Then I created a new char in a new world, same result. Lore page after reading not found in journal.
