I had this happen and this seems an older post than a day or two, but I started playing a couple weeks ago and if you just quit to desktop regardless of where you are, it spawns you back at home.
Point being, there should be a game mechanic to /unstuck yourself from the many world places you can get stuck. This is not a new thing in gaming, been around for years in many different games.
Once you upgrade the flame and have more altars its worth designating a couple of them for exploring so you don't end up with a long walk back if you need to either respawn or fast travel out of being stuck.
Down in the shroud the only option is to drop to the menu and back.
Since the unstuck would probably place you at the last altar/respawn a button might be superfluous but a loading screen tip certainly would hurt.
