How/when do you get to save?
I made it to the first beacon outside the starter cave and it gave some sort of notice that made me think this was a save point. I exited the game and when I started again I was back in the very beginning area. I did have all the items I had picked up.

Did you actually read the entire notification? It clearly informs you that it is a place where you can *build* the core of a base, referred to as a Flame Altar.

Are you playing on a private world? Anything you do is saved pretty much as you do it but when you spawn into the game you don't spawn where you left off at, you spawn at the last flame altar (your base) you were at. So if you didn't drop any altar(s), you would spawn back at the starting spot.

there is no manual save action; it saves as you go and any game restart will put you back at the last visited altar.

Also worth noting because you can DIE for this; there is NO PAUSE. Not really. Oh you can put up the menu and it looks paused, but its not. You are not "safe" from attack/damage in the field while "paused" if something comes along. If you want to be truly safe.... go to your home base altar (then it doesnt matter if you "pause" or not)

I didn't make it to the map marker for the place to build a base. I'm talking about what I thought was labeled as a beacon. It was on the right after you exit the cave. And no I don't remember exactly what it said because I'm old and CRS.

It is an interesting way of doing things, but this game does not save anything outside of your flame altar cube of influence. Any changes you make outside this border will revert to the original form upon each new login, or after a certain amount of time spent away from this location. This is why the tutorial encourages you to get your first altar almost immediately. So you have a place that will not change like this.

It says "If you fall you will rise again at your last return beacon". I assumed this was a return beacon but it wasn't. I have since made a base and have a spawn point.
15 months behind on this (just started Enshrouded today), but I thought I'd throw my hat on OP's side of logic, as that BRIGHT SHINING THING which ironically gives you the tip about respawning at the "last return beacon" (when it could have said -- you know -- "flame altar") and uh: YEAH, I almost took a breather and logged out, thinking that thing was what to which it was referring. Thankfully, I kept going (it's hard to stop, I love it so) and built a flame altar, just in case.

Before you log off find a small spot and build a temporary flame altar. When you log back in you will spawn at that altar. You can then deactivate it and it will go away. You only get some many altars you can build so I honestly save them for farming points to go to :) Good luck!!
