Which flame shrines I've completed?
Some of the markers on the map get a little checkmark when you've finished them, but not the flame shrines, and the shrines show up before you even reach them, so how do I know which ones I've already collected the sparks from?

The flame shrines regenerate so you are able to return again later and get another spark so technically they are never completed the way some of the other things are.

How long does it take to regenerate? I visited an old one recently and it wasn't burning again

The world seems to reset when you logout. Stuff inside a flame altar's build range will never respawn AFAIK.
