Has a campaign / narrative elements, nice progression elements, big bestiary, very great world, etc.

better rent one , that way your server will be 24/24 7/7 online. thats it.
I vouch for this Grounded is a great game especially co-op.
are there NPCS? Not just ones to kill, but quest givers, factions, merchants?
When you say "Rent a server", that sounds a little intimidating. My experience so far with that is there is a complex/arcane set of instructions to configure and set up security. I keep waiting for a company to make it out of the box, you just pick your environment (solo) and pay for it, I believe Fallout 76 does that, but for many reasons that is not the game for us.
I just want a little world her and I can play in, but I do not want to just build houses for nobody and gather resources, for nothing other than a grind of building more empty houses.
I have seen Icarus, it had a lot of item store things that gave me a red flag.
I have seen Grounded, but it's a little too cutesy for me, I like the concept of a high fantasy setting.
Another game to potentially take a look at would be Outward. It's a few years old but sounds a lot like what you're wanting.
