3 is impossible to answer currently. gotta wait for release for that and probaly a fair bit longer afterwards. as its fully unknown how moddable the game engine will be.
2. yes. all you need is another flame altar. these require rarer materials to be made so at the start youre limited to just 1. but you can get more. its the precise same system as V risings castle hearts if you know that game. the altar range can be upgraded aswell.
1. i am not certain on this one. very specific.
the blacksmith kinda unlocks like a new building piece that you just place down at your base and interact with. you need a certain item to place them down. i dont see why you cannot summon him more than once per world tough. so probaly possible. just need the right tools and resources like always. pretty safe to assume all found npcs act like that. npcs unlock based on quest progression. i dont think quest progression is world based. its probaly character based. so aslong as you did the quest you should be able to place the npc down. think of them like crafting stations mainly in how they function.
