Do people even play this game anymore? I don't want to buy it and be the only person.

Take a look at RO2, it is very active, this game however... no so much, maybe one or two servers at most, and then maybe one Mare Nostrum and one Darkest Hour server.

It depends when you can play.
If you are Eastern Asian, it would be pretty difficult (ping+hour),
if you are European or Nth American : good
if you are Sth American : high ping

I live in NA and I can never find a server with more than 4 people :(

For about a year, the best time for the Americas is daytime as they can get on a European server, which is where the busy ones are.

However, I'd recommend just playing Darkest Hour, which you get for free as part of Ostfront. Darkest Hour has an active American Community.
