That happened to me first time the game started too. It's not 21:9 its just stretching it out horizontally. Look at how wide Shantae is. Change it to windowed then back to fullscreen and it will be black bars.

Yeah the option really needs to be there, instead of having to change res and switch it back and forth from windowed to full screen every time.
