During runs, you can only save via quicksaving - there is a quicksave point at the entrance to each map, as well as one in each camp room, so you'll always have access to it (unless you teleport scroll and get placed somewhere with enemies between you and it, I guess). This save is temporary, and will be deleted when you load.
Note that the game does have an active behind-the-scenes saving system, so if the game crashes for some reason, you're less likely to lose your entire run, which is a nice quality-of-life feature.
I understand they are in Early Access, but the developers need to make the save system much more clear in the future. That is pretty important stuff. They should at the very least have an auto-save icon; if they already do, I haven't seen it and it hasn't been explained within the game.
