Bugged quests in act 3: Missing Scouts and Attack out of Nowhere

Both of these worked fine on my first playthrough but are bugged now.

Missing Scouts did not advance when I explored the correct map - I'm trying the kingdom action to see if that works.

Attack out of Nowhere auto-failed when I entered the map that should have completed it (Molten Scar). This *might* be because I did the Ivory Sanctum first this time around?

I have exact same issues with both of those quests. Entering Sarkorian mines and exploring it entirely did not advance the Missing Scouts quest.

Likewise, if I go to the Molten Scar it auto fails the Attack Out of Nowhere quest.

Molten scar was also deserted. I wonder if the idea is that all the cultists bug out when you complete chasing down whats-her-face after cleaning out Ivory Sanctum, so Attack Out of Nowhere fails?

I did the kingdom project for Lost Scouts, and it completed but that quest still didn't advance. That seems like a just plain bug.
