I had the same issue. it's a very nasty bug. I believe it's related to enlarge person (or any size changing spell) and dragon form.
When you use them both in a certain order you can get the enlarge person buff while in dragon form. It usually works out, but in some instances where there is a cutscene before that forces a human transformation, you get transformed back but enlarge person then bugs out and you get locked out of any transformation permanently, even when the spell is not active anymore.
For that fight, my advice is to start the fight unbuffed in human form, and not use size buffs nor dragon form. Since there are cutscenes in that fight that can all mess with the form.
That's what I ended up doing.
I did notice that my character was under an enlarge effect, but I'm not sure which spell it came from. As I mentioned, I'm still a bit of a noob with this type of game.
I'll follow your tip, and if anything else comes up, I'll be back here.
I didn't transform during that battle, and whenever I try elsewhere, my character just puts his hand over his chest (the standard self-targeting spell animation), a bit of blood splatters, and nothing happens.
Guess I'll have to stick with human form after all—I knew I should have taken the Azata path :P
