Report a translation problem
(The actual FOLDER can be named anything and doesn't need to be named "001", it's the names of the files inside that matter.)
Yeah all individual image names on all folders, 001, 002, 003 for example ALL must have these 3 named images based on the size: Fulllength, Medium, Small.
So if you had images with different names, you have to change the names to that inside their folders, it will only see those and yes you need all folders to have 3 versions of the image on those 3 sizes with those 3 exact names, so do not put numbers in the image names. :)
And might need to exit and launch the game again for it to update them from the folder, just in case if you were already in the game while copied the image folders and made sure image file names are all correctly.
Can you take screenshot of your folder and files?
I assume they are the right sizes all the files with right names on the images?

You can use that and follow the instructions it has, if i remember correctly it does make all the right size images from image you have on your drive, so it's sort of easy way for a beginner. :) Just in case, there is a step you have done wrong, so wrong image name or size.
Edit: And yes, i used that notra portrait maker the 1st time ever i used custom portrait, i had made one myself, but the formatting and where and how was mystery to me so that helped and it worked. ^^
And just in case, the folder & images structure should be like exactly like this and the images named on each folder should be the same, those are not example names, they need to be this:
IMG1: Fulllength, Medium, Small
IMG2: Fulllength, Medium, Small
IMG3: Fulllength, Medium, Small
The folder names can be anything, but inside them all must be those 3 same named images in right sizes
Fulllength: 692x1024
Medium: 330x432
Small: 185x242
Those sizes in .png format.
At least have to learn this once, same works in Rogue Trader and Kingmaker too by the way. ^^ Not sure if the sizes are exactly pixel strict, but the names are for sure, gotta be always the same.
Yes, the images are .png
Not have tried jpeg or any others so not sure if works, but .png images work 100%
And if there is some step gone wrong, one last thing to try is just go to nexus mods, take portrait pack like this for example:

And extract it into the right location, it has the right name and file format, so you can then see it and just replicate that with your own custom portait, making it look same as all others in the folder, so easier to do when sees how they should be and look with own eyes. :)
In Portraits folder create a new folder and name it what ever you like and put the three images into it and rename them according to size Fulllength, Medium, and Small so it should look like Fulllength.png, Medium.png and Small.png in the folder that is all you need to do.
well i can't solve the png thing until i can actually get into the game proper lol the loading screen doesn't allow it and there seems to be no fix i can use
You said that when you tried making a custom portrait it was all blank and may of not been .png but jpeg if so have you tried to removing the savegame as that may be the reason.
