When you become lich, your hair will go whitish and your skin become dark grey.
You do not look like a skeleton but you will definitely change.

Stage 2 shows a skeleton body.
You can also choose to not go through the ritual. That way you retain your mortal appearance. But as a side effect you are capped at either mythic rank 7 or 8. One of the two.
For instance: Mythic Lich Damage Any is the skeletal body part model. It's dead easy to switch it out for whatever your race's sex usually uses. And you're not limited to mythic paths or ordinary body parts either. Walking around as a zombie is fun!

So you get to look like Skeletor for the end of Act 5 and Act 6. Most of the game you will not be a skeleton.
Partly due to evil paths getting the short end of the stick plus all evil paths feel forced like you do stuff just for the sake of being evil.
But anyway Act 4 is the most boring experience in the entire game. I get stranded there a lot. That has nothing to do with evil or good, just that the Abyss is utterly boring. It's like a normal human town with awkward pathing and demons instead of people pretending they're bad people. But honestly it all seems super civilized and neatly organized. More than the rest of Golarion in some cases.
If you haven't tried demon path you can pretty easily play that one as chaotic neutral for the most part
Act 4 is one of the best part of the game. It's absolutely fantastic and I love it from start to finish. At least as a Demon (after 1550h of playtime with this game, I've only ever played as a Demon, except for the first playthrough, where I was a Lich, but I didn't complete the run, because, as a Lich, I couldn't kill who I can kill as a Demon and, for an evil character, it doesn't make any sense to accept being controlled and used).
Eh? Isn't that like a huge part of a ton of evil characters? They willingly (more or less) accept being controlled and used by more powerful entities for the promise that they will attain more power for themselves?
I mean, that's pretty much the entire reason devils do what they do, isn't it?
as for the 3d avatar, at like act 8 or so, you'll undergo a ritual and become a skeleton. Not really a choice (unless you'd rather not be a lich).
That said, you could still use transformation (or similar polymorph) to become some other creature, while your base form is skeleton.
And there is a slightly different skeleton depending on your base race, so it's not just everyone becoming a human skeleton.
Alas... there are no different lich portraits to go with the various skeletal race models. Having a human skeleton for a portrait is especially jarring for shorties or kitsune.
I'm tempted to go with a fey shifter because it looks demon like but have no idea if shifter is good for demon
