not understanding this part of combat

says my fortitude saving throw succeeded ...so why do i have ghoul fever and paralysis from the attack then?

some effects have you make more than one save to shake off... look at the effect in your character sheet.... it'll give more detail on the effect... but think ghoul fever has 2 saves... initial save when getting it and think another save 24 hours later.... not 100% sure...

Ghoul fever inflicts 1d3 Con and 1d3 Dex damage. For completely overcoming it you need 2 consecutive saves.

A ghoul's paralysis attack is unrelated to the disease. From the sound of it you succeeded in wading off the disease but still succumbed to their paralysis attack. That one has a separate save.
