1) Download UnityModManager and follow its instructions so you have mod manager in-game.
2) Download "Custom npc portraits" mod and install it with UnityModManager.
3) Open in-game mod manager (ctrl+F10 is default combination to do it)
4) Find "Custom npc portraits" mod tab inside window that popped up. Open it with "sliders" looking icon.
5) Turn off the "Let the mod manage the portraits of main char, companions, and mercenaries." option. (uncheck little x inside a white square next to the text).
Make sure to save the config file, you do it with the button at the bottom of in-game mod manager window, wich says "save".
6) Custom character and companion portraits should work now.
