Report a translation problem
If you're on an island, you should be able to work your way back to the original room, and there will be an icon to go back.
If you're on the ship, go down to the main deck, and about halfway along the side (starboard side, iirc), there will be an icon to go back to the cave. Once in the cave, you just walk out like normal.
I agree it's a bit counterintuitive... I didn't realize for like 12 islands that I could leave an island... but there it is.
The debuff is supposed to go away once you leave the island. So it's a little tedious casting greater restoration on everyone to make sure.
But you're not that far yet.
