I always have her die, sadly I can't kill Anevia without being completely evil so I never do it.

Depends. If you kept your commander title before going in the Abyss at the end of Act 3, Irabeth is in Drezen, she doesn't go to Iz.
Why do you hate her? I like Irabeth, she is not stupid lawful good fanatic Paladin, but much more lawrul than Seelah. She had sucsess with her eaglewatch but stays very human all the time. She was almost broken i the chapel to see all her comrardes tortured and turned into ghuls, she sees her weakness and wanted to get degraded. And you can help her, ro get her faith and selfconfidence back.
You need to be very evil and heartless not to serve her. Irabeth is next to Seelah the most loyal person, you can think of. Unlike a certain jealous Queen..
The Glass key is only important for the Ascension ending FWIW.
Did you complete Irabeth's personal quest? That might have a bearing.
It shouldn't prevent the ending if you add the key in your inventory(At least, I don't think so). However, even without Irabeth, the queen shouldn't die if you go save her before the cutscene with Deskari. Getting the glass key without fighting the enemies in the temple shouldn't trigger it either.
