Does anyone know where to find frozen crescent throwing axe ?

There is a lot of quest guides and build guides but finding a weapon locations is impossible on google search. Wiki is crap, fextralife is crap, There are weapons listed but a lot of them has no location or how to obtain description. I dont want to miss frozen crescent because it is the ONLY +3 throwing axe in game with 18-20 crt range
