Yep, that is my experience as well. Moved on to the next chapter, and the quest to tell him about the death of the Inquisitor is still on my log though...heh...
I'm also not sure about that. I think if you do the Tavern Def first, you can still get the showdown between Hulrun and Ramien. Not sure what happens if you kill Hulrum and don't find Ramien before the tavern defense. In my current game I killed Hulrun, did the Mansion quest on my way back I revisited the Market Square and found Ramien. He definitely wasn't there before (I kept a close log on my combat log to see if I had any failed rolls), so it seems I had to reload the Market Square before finding him.
I found him by the gate and the normal angle made him seem not there. That was alomg time ago tho
