No clue why it wouldn't work on kitsunes in human disguise as it counts as polymorphed for other effects.
I suspect it won't work for magical transformations like dragon form either.
As far as I remember there is one single druid npc in the game but none of the companions qualify.
It is similar to other items that are very restrictive on classes, for instance in the crusade (act 3) you can get a relic which can be enchanted and is quite powerful but only works for monks.
Hmmm updates didnt seem to fix this belt. Playing today on hard mode, belt does not work on Aivu,my paladin's horse or any other animal companion in my party.
