Act II - on your way to Drezen
Act III - Western Worldwound area
Act IV - Abyss
Act V - Entire Worldwound area
Act VI - Endgame

yes whenever the red hourglass quest fails on quest.

So base on that.
act 1: Kenabres
act 2: Assualt on Drezen
act 3: Fighting the demon
act 4: Abyss
act 5: Advancing into the world wound

Here is a list of the Acts final quests - they should give you orientation.

I'd say they're fairly obvious. And the game usually stops to tell you when the quest you're about to do is going to end the chapter.

I forget the names of the quests, but generally:

Act 1: Assaulting the Gray Garrison
Act 2: The Battle of Drezen. Going into the camp outside of Drezen funnels you into the battle, so wait until you're ready to do that last army battle.
Act 3: Assaulting the Midnight Fane.
Act 4: Taking the Airship to Colphyr.
Act 5: Going to Threshold.
