By Iz I had a 52 AC and a cloak that made me invisible every time I killed something. Pretty much waltz through everything from there. Fear is your enemy. If you have no way to combat it you are in trouble. I am a bit annoyed that you only get swarm clones pretty much right before the last fight. I ended the game with a SS of 10, and running around with 4 swarm clones at that level makes the last fight incredibly easy, so that may be why they did it. You would steamroll through everything pretty quickly if you got it earlier.
Swarm infest is your friend. Use it often. Remember to change your XP to only active companions in party right after you turn. It is on the first page of the difficulty screen in the first section. I felt like Swarm was kind of just tacked on. It made me a bit sad, I wish there would have been some swarm specific quests, as you lose pretty much all of the later crusade options. The ending also left a bit to be desired. This is such an outlier class that I hoped they would do more.

But be warned - my run got "bugged" out hard after I made swarm clones and started a new party. I could not select any mythic path at myth 3 on the new run so it forced me to start over.
That bug has been fix apparently. I happened with gold dragon as well. Got the same bug with swarm after starting a new save file. Update to EE fixed it for me so the run was saved.
Swarm's only weakness is elemental damage pretty much, having resist spells helps a lot.
The only hard bit really is finishing your act 5 mythic quest, after that you're pretty much unbeatable.
