Its similar to Kingmaker if you have played it . If an advisor was busy with an event card you had to wait for it to be done before he/she ranks up.
I'm in the same boat.
I have a similar issue with leadership. Eventually the rank up DID pop up , but I am 1 quest away from ending the act and I am still in rank 3 .....
Edit: Did some testing by loading a save before the second rank up event. The first time, I agreed with Konomi's suggestion when she insisted on it. I didn't get the mercenary Sorcerers that I was supposed to get by choosing to ally with Isger. This time I told her that I trust my advisors more than the royal council and chose Lastwall instead. This time I did get the Inquisitors I was supposed to get from this decision.
I suspect just agreeing with Konomi when she interrupted me while I was asking my advisors' opinions broke the event. Going to test if avoiding this allows the third rank up event to fire properly.
