So yes every successfully timed input leads to combo points. Every weapon is different timing, some of the great hammers seem like they dont want to combo off a heavy attack at all. To my knowledge you can't combo a heavy to a heavy. You can do Heavy light. Or light light, and chain as many lights as you want on the end. I could be wrong and maybe you can do light heavy. But you get the idea. practice swinging every new weapon you try for a couple mins before going out with it.
As for how to tell if you're getting them right, there may be a better way to tell, but I just watch my stamina while I practice- between that and the noticeably 'smoother' feel of a properly done combo, you can usually tell the difference from my experience.
Maybe there's a better way to know for sure, but that's what's worked for me.
There are indicators on the bottom left of the screen.
Worked so far with the weapons I tried while watching my stamina... It is actually easy to find a rythm if you tried couple of times. I tend to screw up when I get charged by a giant spider with a human face tho :'D
