[Bug][Quest] Into the Pit unable to progress & Icon Issues

I have entered the portal for the quest Into the Pit and completed it successfully three times now and the quest is not registering the progress. I have also experienced a number of times where my quests are on my sidebar tracker but none of the quest icons for where to go are visible on the map or in my characters field of view. I need to close and reopen the game to get the quest icons to show back up.
