NPC cities / regions fight?
was wondering if the NPC cities and regions actively fight each other or are they just there sitting doing nothing?

Yes. From what I understand, the different factions are indeed simulated. They can wage war with one another and suffer political strife. If you go to the compass-looking icon in the top-right, it is known as the "World Log." It details the simulated actions/timeline of events for the NPC factions in a similar way to Dwarf Fortress's generation of the world. Granted, I do not remember if DF has its world events also occur in Fortress Mode.
One of my neighbors apparently broke apart due to "internal struggles," for example. I'm not sure if this means that a new faction was made (a-la rebels) or if a province has become independent, etc.

depending on what you mean by world events DF does do that during gameplay too! I just got off of a DF binge and im looking into this game lol

Thanks! I genuinely have not played DF for quite some time now, but I do remember like monarchs dying and stuff.

Yeah, having played both, it's kind of interesting to compare the two! I love DF's depth. I love the fact that religions are generated, civilizations are formed, and an entire history is ready for you in a few minutes. :)
Modding it is also super easy because you can literally change the game with just a .txt document (it's how I got papyrus to spawn outside of swamps for a crop dedicated to papermaking, for example). It's just so crazy that I can zoom-in on a character and get their name, backstory, traits, personality, and relationships right there.
I like Songs of Syx because it's (in my opinion) focused around the part that I mostly played DF for: the city-building.

Ok. So. In the "vanilla" folder > vanilla plants > objects > plant_crops.txt there is the "Papyrus" crop. Like it real life, it grows in Tropical Wetlands (think Egypt's Nile River delta for the civilization's paper making). All you have to do is change it to all tropical biomes. :) That means that your civilization/expedition can now select it from embark (I believe) and that you can forage it when you arrive to, like, a tropical rainforest (where I first assumed that it would be but no it's in wetlands only). Please see the copy-paste code for reference (The biome is changed to [BIOME:ANY_TROPICAL].) :)
