@OP - Right click on the game in your Steam Library, Manage -> Browse Local Files
In this folder, make sure you have file extensions turned on by clicking the View button on the top menu -> Show -> File name extensions
Create an empty text file named imma
Right click on this new text file and click Properties
You should see the name of the file in Properties as imma.txt (if you can't see the .txt part you need to make sure file name extensions is turned on)
Rename the file as imma.degenerate - your computer will ask you if you want to change the file type, click Yes

That should enable Incest - just make sure the file is named exactly as imma.degenerate at the end

And yes it works for me. This is all I had to do. If you really want I'll hop in-game and show a screenshot of it working - there's different text if you successfully mate parents who are siblings.

Works on my machine.
Open the game folder, open cmd in it (you can type cmd in explorer's address bar and hit enter), and run

echo. > imma.degenerate

to just make the file without having to deal with gui nonsense.

You have to enable file extensions. Click View -> Show -> File Name Extensions.

You should then be able to rename the file with the .degenerate file type.

even more, after that incest enable thing, it said: "It will not turn out well..."
And, " 'Ur character's name' and 'Mate's(ur offspring's) name' have kept it in the family..." (kinda like a pun on the family tree in a form of the circles lol)
