The meta for building a strong dynasty is to land as many dynasty members as possible. Otherwise, unlanded branches of your family are likely to simply die out.
Without an heir, the county reverts to you if they die.

Edit: Mechanically speaking, being homosexual has no bearing on fertility in this game compared to CK2. It only affects opinion and schemes.
P.S. I did the math for you on your scenario assuming the wife had no prior children and they own no artifacts. Roughly 2% chance a month for them to get pregnant. If they got a laying event from each other or the wife's lover it would be 100%.
I killed there kid and literally within a couple months she was pregnant again.
He's 74 and homosexual and she's 46 now. Isn't this actually breaking the rules of the game now?
I haven't specifically looked at that scripting, but it probably goes to ~45 (for game reasons), at witch point something like an "if" statement would prevent it from occurring despite fertility being high.
Then again it could be implemented differently then that, because maybe it has some downsides in the way I mentioned or they wanted something else.
IIRC men can have children till 75.
-Without modding the game, women in CK3 cannot get pregnant past "46". None of this "early 50's" thing.
-Women are not set to "early 40's", but hard menopause at age 45. I've witnessed pregnancies at age 46, but this is because they got knocked up when they were still 45.
All of this can be confirmed in the NChildbirth section of 00_defines.txt in your game files.
Irrelevant. So far as I can tell, sexual orientation has no direct effect on fertility.
Woman with Fecund trait can have children at the max age of 50.
To OP: Giving land to rule removes -50% fertility reduction that happens to all landless characters. And probably her lover knocked her up immediately. Go to debug mode and take a peek.
