The DLCs are all the good toppings.
Some people will be satisfied with just the simple pizza, but if you want the real good meal you need the toppings.
Its your choice in the end.
Some of the DLC is terrible, like the struggle systems. The rest make the early game more interesting, but the late game bloated and you are left overpowered, whack-a-moleing various events and swapping out inventory items constantly.
Overall, its fine, but again, way too expensive.
Cosmetics are optional :)
TL;DR Yeah it's worth it.
Fate of Iberia and Legacies of the Dead are the only two DLCs that actively make the experience worse.
The rest: depends on your budget. I have nu ragerts, but I'm an imbecile and waste money on garbage.
The base game is cool enough for what i'm doing with it.
I just play the AGOT mod anyway.
Yes I recommend them all I think a lot of these people are just negative nancys some are about price and that's fine but a lot of it is just negative to be negative. Kinda like the WoW community was always hyper complainers. My take. I'd get the bundles which did save me a good amount I think. So what I read in those same comments or reviews didn't equate to what I'm experiencing in game. My two cents.
Moreover, said complaints are often outdated after a couple of patches, and/or mistakenly misattribute free patch features, like diseases, to DLC content. That is to say, reviews of Paradox DLCs tend to be useless; you're better off just using your own judgement based off of what is on offer.
The game is never made better by removing a DLC from the game that you already own. So yes, all DLC make the game better, even if not all of them make it substantially better.
Comments blending an update complaint to the base game with the DLC with the DLC itself are just... inane. Those 'changes' are there with or without the DLC. Without the DLC the changed or new mechanic might be harder to work with - but this isn't a game where you have to grind for hours if you don't get the DLC. You just work it differently (or mod).
Too expensive for the content involved? Wait for a sale.
Too 'buggy'? It's PDX. Again, wait a while after release. They fix the worst bugs, and balance a little, eventually.
The content 'should of been in the base game'? Obviously it wasn't. The base game was and is playable without it.
Regional content is just that. If you're only ever going to play in India Northern Lords and Fate of Iberia are going to feel like a waste of money.
None of them 'worsen' gameplay. Anything there that is complained about being there is base game without the DLC too.
