I'm not sure any DLC is worth it without being on sale, but the smaller purchases are maybe the most good as compared to Royal Court (which is good, but also introduces a lot of User Interface h377).
Since Friends and Foes doesn't introduce a lot of bad, I'd say it's worth adding to your CK3 collection if you enjoy the game.
I believe it's something that mods will put to good use when they get to it.
If you like immersion, roleplay, then yes, get the dlc.
If you dont give a damn about roleplay and immersion, and just want to paint the map your colour, then I would bother getting the dlc.
