Its really not the trait hoosexual. Ive plyed plenty gay rulers in ck3 and got dozens of kids most time. Its a bit luck from time to time. Choose same lifestyle traits to counter it, there are plenty with fertility bonusses to help your out, but also check your wife! Its a bit silly the system. The fertility penalty should fire after birth of first son, as it is no healthy thing just pure overcoming for the dynasty. But like i said nevver got those problems in the game, as there are way to many fertility bonusses.

My homosexual leader still managed to have 3 kids. They're not sterile, just the odds are lower - and having fewer kids can actually be a good thing. You could try befriending or romancing the wife to improve the odds.

My realm is pretty safe from splintering, cause I only got one kingdom title and only expands as much so no other kingdom title can be created.

Lol one ruler of mine got 12 kids. Only when he died i found out he was homosexual. I wonder how many of his kids were his haha ;)

Not only can your homosexual leader have kids, he can even have kids if the spouse is homosexual as well. I married my homosexual son to a homosexual wife by mistake as I didn't pay attention to their sexual orientation. However they ended up having 5 children together despite both being homosexual. Also none of them had any +fertility trait. So it's definitely possible.
