Wrong holding of temples as administrative gov
I reformed to a heretical religion, making myself the head, and now many of my temple holdings will not provide any levies, taxes, or plague resistances. The problem is that this isn't consistent, and there are plenty of examples of temples providing all three. I cannot see a pattern between religion and culture of the person assigned, to diagnose the cause precisely, so I'm not reforming again unnecessarily.

Am I supposed to switch back to a revocable spiritual leader? Any workarounds? The control of holy wars isn't worth losing plague protection.

Temple holdings aren't determined by government type (unless you're using a mod), they are determined by whether the faith uses Lay Clergy or not. If the Faith has Lay Clergy then any one can hold a temple ( and will use whatever system of government their liege uses by default) but if it doesn't then Temples are automatically leased out to your Realm Priest who will appoint ( Theocratic ) holders. This can get weird, if you've just done a faith change, because it theoretically checks the Faith of the holder of the Temple barony but the default holder if none exists is the County holder.

This is interesting. And I'm confused. My own counties. Roma collects taxes and levies and recruits a lay clergy from my new religion when converted counties in my home duchy can only recruit orthodox and cannot collect taxes. I have other orthodox tenants that do collect taxes, and represent most of them.

I have not revoked the orthodox patriarch, should that be done, like the pope?

If the Orthodox Temple in your duchy is in a county that you directly control, you should be able to pause the game and revoke the Temple barony title and appoint someone else. If the county is controlled by someone else, then you may need to check if they converted and demand their conversion if they didn't or revoke their title otherwise. They should clean up any wrong faith temples in their domain eventually though it might take a while.

Revoking the Orthodox Patriarch won't change anything ( Temples still operate like this if a faith has non-Lay Clergy and no Head of Faith at all ).

It's curious, I have revoked all of them multiple times and and the only option is to lease to orthodox, despite the county already being converted. I can collect rents and use levies from orthodox tenants in Roma temple holdings, which I own and is also converted already. It's just confusing.

The county faith doesn't matter, it's the faith of the person who holds the county title that's checked.
