You may have issues with some of those added mechanics without the DLC complicating the base game, but there's usually a mod workaround, addition, or flat out cheat that can be downloaded.
Much shorter answer: Yes.
If you're on the fence, wishlist it and wait for a sale.
Yes thousands and thousands and thousands played it on release and loved it. You get all the free patches since release. Well worth 19 USD
I paid maybe 100 bucks or so since release and played 1600 hours, that is hard to beat.
Remember your 2 hours for refund! Although that is just a little time to look around.
It's a Paradox game. There's no way to even figure out how to really play it in two hours.

Yes. You don't need any of the DLC. They are just nice to haves, and fun too. But the base game is sufficient. There's many mods that provide OCEANS of better and far more relevant and interesting content and mechanics that what the DLC's do.
It's not always like that with Paradox' games. For example Hoi4 has some DLC's that enables some very core mechanics and content that I feel should have been in the base game and which annoyed when I was playing it without those DLC. But with CK3 I think the base game is absolutely fine; go look into some of the amazing mods the community have made that REALLY makes a huge difference.
I have all the DLC, but tbh, still haven't used everything it adds.
For instance hybrid cultures came free, and diverging (altering a culture) and creating your own cultures in game are DLC (although someone could simple create a culture in a mod.)
I have only ever used diverge culture once in hundreds of hours. Others use it all the time. Thing is, knowing what you want will tell you if the DLC is needed.
Hybridization naturally occurs and you can even encourage it. So it's there. Just not all the extensions you could do in the DLC.
Haha yeah it was hard to keep a straight face with that one when I said it! He will look up and 5 hours are gone if he likes it.
