2. Assign him to lead a small army doomed to fail and send him off to die in battle or get captured & executed.
3. Disinherit him, this costs renown and I think some prestige
4. Click on your primary title and establish a new law; this will cost a lot of prestige but will let vassals vote on who will inherit the title, then you can enforce votes etc. Of course this is expensive and risky, as someone else entirely could get the title if you can't secure the vote.
2nd - I just got done putting my primary heir (who didn't want to become a monk, despite have all super awesome monky traits) in front of my levies (making sure him as a knight was in the unit AND leading the unit. Then sending him in with my elite unit behind him just enough to arrive in time. Needless to say, after 3 battles, he perished.
3rd - Dungeon, but this is risky as some kids just inherently live long in a dungeon. Add some torture for flavor?
4th and probably the easiest, just murder the guy.
This list could probably go on quite a ways, but I think these 4 are tops.

i solved it when i hybridized my culture with han culture and got Eunichs.
basically you just cut off as many balls as you need until you son of choice becomes your heir.
according to wiki you need either a Communal, Ceremonious or Spiritual ethos if you want to reform your culture with court eunichs.
i havnt tried this yet but maybe you can also put him into an army and then park him in some epidemic center?`

actually second time he finally died, and what happened?
My 2 years old third son become my heir, instead of my 19 years old second born wtf..
What to do now?
Educate the 2 year old the best you can.
(so you'll have to murder the kids, too)
It's odd but, the game sometimes glitches and shows what it thinks is the next heir but, after a bit it adjust OR log out log back and it's fixed.
