Just get a good physician. don't worry so much about others dying just keep your family and your best courtiers alive. even if you are Emperor or King.. even if everybody hates you.
Who cares your life and your dynasty comes first.
You don't need to play on lowers difficulty diseases. just learn that your character and family comes first.
I feel like it depends on geography/topoghraphy. When I tried playing in the Balkans it felt like I was getting smacked with a disease every 5 or so years. My family and I would survive, but those disease resistance modifiers and upgrades didn't seem like they did much to prevent the spread and just prevented death.
In my current Tibet campaign, I've had maybe, 1 disease roll in from India. Other than that, I've pretty much been disease free.
