Seeking Realistic Gameplay Campaign Settings: What Elements Enhance Immersion?

I have the following settings in Crusader Kings 3:

1 Exclave Independence: Total
2 Matrilineal Marriage: Never
3 Diplomatic Range: Restricted
4 De Jure Requirement: On

I also have a couple of questions about the game:

1 Random Nickname Frequency: Is it worth keeping it on default or doubling it?
2 Random Harm settings: Is the "Dangerous" option sufficient in terms of content and the risk of dying?

The harm thing is still up for debate but IMO it feels a bit over-tuned at the moment even at the lowest settings (excluding safe which kinda defeats the purpose because it *should* be possible to die of a freak accident but maybe never seeing the same event within weeks of the last occurrence, but that's probably just due to a lack of variety so hopefully they have the writers coming up with fresh events).

I also turned cultural/religious conversion to the slowest settings because otherwise you end up with cultures flipping back and forth a bit too quickly with conquests/inheritances, but that's probably more personal taste. I just don't think everyone in a county would keep changing back and forth every 10 years.
