Can someone explain regency to me?

I thought I understood regency, but every time I have a selfless vassal who takes control for whatever reason, off on a pilgrimage or child inheriting etc they immediately become self-interested and I have an awful time with scales of power etc.

Every single time, every game, multiple instances.

Is it me misunderstanding something or is this broken?

Get the Talent Tradition, get an Vassal with an Criminal Secret, expose it and forgive him and now, you have an Regent, who is forced to be loyal, because of your Loyalty Hook against him.

Make your Regent somebody you have a Strong Hook against. This also has a cool roleplay vibe to it.

Strong Hooks, don't guarantee Loyalty, only the Loyalty Hook.
