How can I create a custom kingdom/empire with the console and add duchies to it?
I can create a custom kingdom with
"effect = { title:k_<your duchy title> = {create_custom_kingdom_effect = title:k_<new kingdom title>}"
Normally you would add a duchy with
"effect = { title:c_<county_ID> = { set_de_jure_liege_title = title:d_<duchy_ID> } }"

Unfortunatly, newly created kingdoms (doesn't matter if the decision or the command was used) have x_x_number ids and not k_kingdomid ids.

For some reasean the second command line doesn't work with these x_x_number ids.
Does anyone know a workaround?

You can edit the save game for adding duchies to kingdoms and kingdoms to empires.

I think I tried that once and the duchies still had the "not rightfull liege" modifier after adding them to my kingdom.

You must edit the duchy and the kingdom.

There are two mods that can help you create and reorder titles as needed:
Ecsaty's Titular Title Creator This one allows you to create new titles.
Title Manager: that one allows to change the de jure hierachy of titles
