Where is the "Unite the Spanish Thrones" decision now?

I have the kingdom title for Castille, Aragon, and Navarra but the decision is not on the list. Has it been moved?

You are confusing creating Hispania and a specific kingdom decision, but holding the titles is not enough, you need all their de jure counties territory as well. Also, what culture are you? You need to have Iberian heritage to take the decision.

Additionally, according to the wiki the start date also has to be 1066 so if you started in 867 the decision does not exist.

Before the update, I remember that you needed to have the kingdoms of Castilla, León and Galicia to take the Unite the Spanish Trones decision.

Before the update, the decision appeared even if you couldn't make it. If it doesn't appear now, it may have been removed.

It is still there for me and it specifically says you need your culture to be an Iberian pillar (which is its heritage) aside from the kingdom requirements, which is why I believe OP might not have the correct culture.

What start date did you choose? Are you playing a custom ruler? Are you culturally Iberian?

The decision exist only on 1066.

I started in 1066, Castilian culture, mozarabic religion. Started with Urraca Jimenez so now I'm a few generations in.

It is already there if you start the game with any of the Jimenez brothers. So you don't need to have the High Medieval Era or High/Max Crown Authority to see it.

Not mentioned in the wiki:

1) You cannot have an empire already (this removes the decision permanently)

highest_held_title_tier < tier_empire

2) At least 2 of the titles to be united must have de jure land left (so if they drifted into your primary title, no more chance)

Apologies, I was confused on which kingdom titles were needed to the decision. Aragon and Navarra don't count for it. I also forgot I had did it with a previous character. Thanks all.
