Everyone dying in their sleep
So my wife, kids, courtiers, etc keep dying in their sleep in their 30s-40s every so often, never saw it happen so much, is something wrong?

Someone in your court is a serial killer.

It's a multiple staged event.
When picking the right solutions you will find out who did it.

So, yes there is something wrong!
No, you cannot prevent it and you, yourself, did nothing wrong.

as above , had one run where my sister killed my wife and my 3 boys in a matter of 5 years

Could be the rare serial killer event. Last time I got that it obliterated my family and squad of knights.

I've had this event a couple times, the serial killer was always my heir. When it happened again, I moved him to his own court and the problem went away. I even got a popup saying I realized the problem mysteriously resolved as soon as he left my court.

Are you modded? That's a common bug/occurrence with some mods, sadly.

I had the serial killer event my first time playing Fate of Iberia... haven't been back to that peninsula since... Like, I wifed THREE times, and each time my wife was targeted and killed... I just was not motivated to continue the game despite my first wife having twins (1 boy 1 girl).

Check your court and see who is sadistic and other like personalities. Normallly they are the silent killers.
Sometimes they sure are usefull to whipe my useless courtiers

I had this problem with my Heir once - "Why you little!"

If your Ruler has both the insane and sadistic traits it would be great if they could be exposed as a killer, without the player's prior awareness.

My current ruler has Sadistic and Lunatic.
