One damn count, that you fail to spot, can spread his filth a long way. If a neighbouring king or an emperor converts, you may even be out of luck.
Only thing i see as a real problem, when the heresies are non-hostile so you can't revoke vassals or holy war neighbors. Like Orthodoxy in France should never happen from a game balance perspective.
Note that only abrahamic faiths can have heresies to begin with, so e.g. the eastern faiths who are always "astray" to each other will never have the problem.

Adamite spreads much faster because it has a +50% conversion rate for "same culture." Guess what, literally every single AI ruler is same culture as their counties. If they don't start that way they are within 5 years. Only time this isn't the case is kings where still 60%+ of their realm is their culture, and they still convert those parts so fast that it doesn't matter. Duke and Count level its 100% guaranteed. County resistance to heresy conversion is absurdly unrealistically ahistorically low.
My experience, too, is that there is simply too much Adamitism in the game, where there isn't so much Bogomilism or Catharism or Lollardy, though I don't know to what extent this is subjective and accidental and to what extent is objective and stable.
My general observation is that Paradox struggles with figuring out or implementing the right MTTHs for the variuos things in the game (events, checks, etc.) and other numerical aspects of balancing. Personally, I wish the developers cared more about the end result and felt obliged to fix the balancing when things get out of hand.
