This is quite weird, I've never seen it before. But who got your lands in Italy?
I think it's happened to me, with major titles on the other side (i.e where your Primary title is? That matters, I think.) So I kept a couple minor things on one side of the West Francia/Aquitaine border, and more of my domains went over to the other side.
Then, there was the time I was playing Otto of Angria, and my Liege lost the Karling consolidation wars... BOOM, game over. All my titles were gone/revoked, and my lands were given to vassals of Charles the Bald. Good times.
The lands I lost this time were in Italy. I did notice that there was other land that was not mine that also went independent at the same time near venice but who knows why...
Another weird thing was that i was put under the king of Frisia instead of West Frankia which made for some nice border gore.
I ended up getting a foothold in Navara and retaking the lands in southern Italy so not the end of the world but also not really cool being jerked around and not knowing I'm about to lose a ton of land either.
Next (new) game set it a bit looser or turn it off (for yourself or completely).
Losing some counties is not the end of the world, you'll survive :)
It is not the end of the world but it ruins a good part of the game and all the carefully planned succession was in vain,
In a sense, getting rid of the empires is not a bad thing, it makes the game interesting again.
One of the ways they did that was through this mechanic.
What I tend to try is to give those counties to extended family.
You will still lose those counties but they stay inside the dynasty.
It is a cheap way to somewhat lesser the pain.
You could always try to hop counties and establish some wayward connection.
(me saying "it's not the end of the world" may have come across a little negative, but that was not my intention)

Basically I was planning for that land to go to my second son when my character died and sortof see what happens to him. I would have given it to him sooner if I had known the land was going to get split off.
