An Insular Pope in Rome?
I play as an Insular Christian and I want to form a new faith with a Spiritual Head of Faith. If I were to conquer Romagna could I install my new head of faith in Rome in something akin to the Papalcy?

Nope.

Any custom faith will be a heresy, you lose the ecumenism doctrine.

Also Rome is only connected to the catholic pope and the papacy.

There's a decision where you can disband the papacy if you own all the Christian holy sites.

If you have a spiritual head of faith, no. They're independent rulers and thus can't be granted land. If you have a temporal head of faith, however, you can grant the head of faith title to the count of Rome, which will then leave you with a head of faith residing in Rome.
