If I'm understanding the pertinent scripting, there's a chance to recover once a year. There's also a "max" recovery time of 30 years, but I'm not sure what that handles.
I've only seen a character recover from it once. Funny enough, that was an heir I had disinherited a few months before they recovered.
in the game I haven't seen gout get cured.

I'm not sure if it "cures" it so much as makes the symptoms less intense.
In any case, all these people telling you to "just kill" your heir are a bit hair-triggered. Give it some time. Let him come of age and see how it affects his health. If at that point you're not pleased, disinherit him and let your second son rule. One of my finest monarchs had gout for a decade and a half before he died at 73, so gout isn't exactly a king-killer.
this is harsh but its cure that (in bad consequence)
Diet is a big part of it, though there are some other things that can contribute as well. So should talk to a doctor if you have it. Though yeah, there is no reason most people have to live with gout and it can be treated. In fact it can leave to permanent damage if left untreated so shouldn't just leave it alone.

Sorry, gout is not funny, it's effing painful and an old mate once said he considered such drastic action to relieve himself of it.
