I had the same problem and the same idea. He never used the hook and died of old age. I couldn't find anything on Google to fix. I can only assume your hook on him discourages him from using his on you. Sorry I can't be more useful.
Anyway I never got rid of that miserable muppet until he died. I haven't bothered joining a witch coven again ever since - the health benefits that still felt pretty random and heavily rolled against where no worth having to deal with some prick having a strong hook on you.
I ALWAYS become a witch and convert all my family but have never had any problem as it is not a crime.
In regard to the health benefit not being worth the hassle, that is not the major benefit; the major benefit is that every single time you attend a coven meeting (grand rite) you get an opportunity to gain a perk (not only in your chosen field), become a herbalist etc.
When you have a massive coven you often get invited to other witches rites so this can happen each year or more frequently (you can only hold your own every 5 years or so); it is a massive boost to stats!
I wish there was a way even if your dynasty dies out you could track how far your witch coven spreads till the end of the game.
